I would go with a simple test cyp and deca stack.....deca will lubricate your joints and test will prevent you from getting deca dick. you will also need an AI during your cycle and pct for when you come off gear. what ever you do, keep the testosterone about 200 mgs a above the deca weekly.
I am not sure if there is a maintainence dose that you can take forever. I would assume you have to come off sometimes. So I am not sure there is a TRT/HRT dose you can take for your shoulder. Deca is a harsh drug so you need time off from it.
