far from an expert, but personally I would keep cardio to a minimum until your natural test levels come back up, muscle loss might be minor but its only a few weeks and I wouldn't chance it

Dude I would listen to your body. If smoking causes anxiety maybe there is a reason. As far as cardio during pct affecting your gains it just depends on your metabolism and protein intake. Keep your protein high and your lifts heavy and you should be g2g. Again everyones metabolism is different though so you may have to adjust the length and number of days of cardio accordingly. Also if you are having polycythemia then donate some blood.

This is right on. The only time I don't do cardio is during PCT.

Your levels of androgens should be at or above physiologic levels up until you are scheduled for PCT, that is if you have timed it correctly. The "delicate" time is when androgen levels far further and high endocrine flux is the state. Generally PCT and the 8 weeks afterward are the most delicate.

It all depends on you. I always get blood work done 60 days post-cycle to see where I am at. That's the only way to know for sure how your body is recovering.

You don't have to to stop running IGF to start pct either.
