The ratios should just be a gauge. They, themselves are not that important providing you have adequate protein and adequate EFAs.
Start with 12kcals per lb of bodyweight, use it for a week and see how your weight changes (bearing in mind any glyogen loss if you're reducing carbs).
Aim for 0.5-1.5lbs loss a week. Anymore and increase calories. Any less and reduce calories.

Depends how much more you've gained/lost.
You need to estimate water/glycogen loss/gain when making the transition also.

Just expect to lose about 5-10lbs more than you'd expect.

If you have access to a bioelectrical impedance Bodyfat calculator, you can get a good idea of whether or not you are glycogen depleted. Test your bf% every day for as many days as it takes to stop dropping significantly. Always at the same time in the am. You will see a fairly quick drop in % and this will most likely be due to water loss which is attributable to glycogen loss. Now, carb up for a day and check it again. It should jump up to near what it was on the first day. This range will give you a general idea as to when you are glycogen depleted. I could give you the theory behind it if you like. Note that this will only work if you are using a low carb diet.
