No idea what 18 stone is, but tell him to count his calories every day. It's a good way to keep track of your intake and really slaps you in the face when you fuck up.

Make him purge every damn piece of candy and junk food from the house. Anything "convenient" (cookies, candy, chips, etc) MUST go.

Tell him to keep a bag of oranges on hand. If he gets the irresistable urge to eat, he can grag an orange. You see, and orange is NOT convenient. You have to think about what you are doing while you peel it. This is a great tool to raise awareness of your snacking habits and keep your sugar intake in check.

This is not the answer to your friends problems but it is a small tool that can help him along.

    Good luck.

Take it slow or he will quit.

    Just like Witmaster said:

    Quit all candy and sweets
If he drinks sodas, have him go to diet sodas
If he drinks whole milk go to skim milk
If he eats chips have him snack on something better
Start walking every day and start a moderate weight program

and be patient because he won't lose it in a couple months.

The last thing you want a over overweight, unfit person to do is start intense exercise. You have to build him back up slowly.

The best thing he can do at the moment is sort out his food. Although not all obese people eat a lot of food (often they can be eating very little in comparison to their size) he might not be eating the right things. So it is always a sensible idea to get them away from the bad food and onto healthy eating. Decrease and stop the icky soda, candy, pizza, bugers/fries and other calorie laden nasty stuff. Work in things such as fresh vegetables and fruits, lean meats, nuts and seeds, wholegrains, legumes and skim dairy.

He doesn't need to start a BB diet with precise ratio's and the like - clean, consistent eating is the key!

Exercise is going to be fundimental at all - get him walking. This is the best thing he can do to start. Swimming or cycling are also good - they are low impact and will help his joints until he gets some of the weight off.

Once he gets a little fitter the intensity can be increased and some form of resistance training should be added in...

Even if he makes small changes there can be marked improvements.

Remember that weight loss is mainly in the diet, not necessarily exercise.
