It's usually worked out from a combination of fecal, skin, sweat, hair and nail losses - although i'm not sure how they measure the nitrogen - probably with some sort of indicator or isotope? (that's what they're called right? or am i getting mixed up with the things on maps? or do they share the same name? Answers on a postcard to....)
You also need to know how much nitrogen (protein) is going in.
Usually, it also depends on calorie intake.
