Ketosis & Carb-up

It has been a month since I have been doing the high protein/mod fat/low carb diet, since 3weeks I am on 10g carbs, in the beginning it was hard, but now I feel great, I have more strength and energy.
I am still confused over few things. The first is WHY DO YOU NEED A CARB UP, when your body is burning fat as fuel, can't you train and stay in ketosis without a carb up?
If you are not training (on a vacation for a week or so), do you still need a carb-up?
Doesn't your body go out of ketosis having more than 30g/60g of carbs, so when you carb up, what happens than? and what happens to the large amount of insuline spike after the carb up?
Can't a bodybuilder achieve his/her goals without a carb up? Dear Dr.Atkin would say YES
