It looks staged because if those guys were going to jump them they wouldn't just smack the girl and stand there. They both would have jumped the guy first and thrown more than one punch.

I think it's real and the two thugs were drunk and or crackheads which explains the slow reaction time. The hits the guy threw were real, and you wouldn't want to stage something were you're getting hit with real punches.
