Here is myself doing 285lbs for 6 reps. Can I please get some opinions on my form?

When you begin to come up at the bottom of the squat, you have a bit of a forward lean. Looks like it is pretty consistent through all six reps. Work on staying back as you come out of the squat, and have somone check your form from the side as you lift.

What MDR said is correct, looks like maybe the bar is a bit high up on your back causing the lean. Also, you are wasting a ton of energy after unracking the weight, stepping back and getting ready to do the first rep. I take one step back into my foot placement and go. Holding that weight on your back that long wastes valuable energy.

Looked good, but I'm old and watching you go that deep hurt my knees..... I use a short chair or another bench and only go to 90 degrees, and when I put on the weight, 285-405 I need the wraps. Just some food for thought for you as you get older.

I'm impressed you can go that deep with 285. Nicely done.

Like others have said, by the 2nd rep you have that wobble forward in order to get out of the bottom. Maybe the weight belt is actually inhibiting your ability of your back to curve properly and therefore your back has to move forward. Do you need to use the belt?
Today I can do what others will not so that tomorrow I will do what others cannot.

The difference between winners and losers is that winners do things that losers don't want to do.

Looks decent. Should always have a spotter tho? Focus on keeping your weight pinned on your heels. It'll give your better sense of center gravity. Good stuff.

Due to reoccuring back injuries, unfortunately I must use the belt when attempting new personal records.

I will definitely try better to make sure I don't lean forward next time.
    Thank you for your opinions.

My knees right now are actually in a lot of pain. So I might just try using the wraps. It's either that or take time off.
