Cold simply means fresh (in the morning preferably), not worked out. So you can flex once and be "cold" for measurement.
I can only assume you didn't measure correctly last time? Dunno otherwise.
I've been training for 3 or 4 months now, and my cold biceps haven't grown. However flexed, they have increased about an inch. Why is this..?

I think he means unflexed size compared to flexed size. Probably had more fat then Mindless... or something along those lines. Or perhaps its something like your ability to recruit muscle fibers has increased so your flex is fuller. But I'm not really sure, somebody else probably knows.
