Check your form (or better yet, get someone else to check your form). Usually when this happens to me, it is because I have gotten carried away increasing my weights in increments, and I am lifting just a tad too heavy and compensating with sloppy joint angles.
Dropping the weight a fair bit, doing more reps very strictly, and getting chummy with the mirror for a while help get me back on track (I train alone at home, so don't usually have the "spotter" option. When available, though, an experienced outside eye is better and faster.)
