I want to keep track of my workouts for various reasons in some kind of easily readable, organized format, but I'm extremely disorganized.

Does anyone have a good example of some kind of organized workout tracking form in PDF format (or a word doc, or whatever) that I could print out and take with me to keep my journal organized and easy to read? I know if i start writing randomly in a notebook by hand, before long it'll be impossible to read.
