500mg/wk for 12 weeks. Use HCG 250 or 500iu 2x/wk up until last pin. PCT with Clomid and Nolva is fine. Up the Clomid to 100/100/50/50(maybe a 5th wk also).
Regarding HCG use, sure you can probably not use it and let your balls shrivel to raisins and then worry about fully bringing them...
Always wear footwear. Just changing shoes once I picked up some nasty foot fungi (like athlete's foot on testosterone). I can only imagine the fungus evolving in gym locker rooms.

This is good info. I've been thinking of something similar for shoulder issues, but what you said makes me think it could be a dangerous combo to 'mask' a shoulder issue when bulking on test.

In my opinion you should always be doing cardio, just varying intensity based on your goals. Steady state works for me, but I hate running.
