I did a methadrol extreme cycle back in mid february. For PCT I only did 10 days of nolva (didnt have any more on hand). Anyways I've been breaking out pretty bad on my back/shoulder's since mid/end of my cycle and it hasnt stopped... I usually NEVER have any acne/breakouts. Right now I have 20 20mg nolva on hand, would it help to do another "pct" in theory now? What can I do to fix this, I have a feeling it's all hormone related.

For me, PCT helps me recover a normal hormone profile, but I actually see more acne post-PCT than I ever do on-cycle. I think more experienced guys can chime on on whether another PCT could do anything to help you. You didn't really run a proper PCT, and this may or may not be related to your acne, but has the potential to cause other issues. In my personal case, as far as post-cycle acne is concerned, I have had great success with Acnedren, from Black Lion Research.

Black Lion Research has a product for this called achnedren
Theyre a sponsor here and the product looks good

You didn't run a proper PCT in my opinion. 10 days of aromasin would have been better in my opinion. Check out aromasin (exemestane), or formeron.

However, 4 months on (how many weeks exactly, 18 weeks later?) is way too long for symptoms. I'm guessing it's something else entirely.

accutane is the last resort. I would try vitamin b-5 in high dose (250mg 3x a day), and at the same time rub benzoyl peroxide on the acne sites, and right before showers rub salicylic acid on the acne and rinse it off in the shower. Keep showers short and try not to use hot water. The oil is what the acne bacteria lives on.
