How do you know he didn't lose one or more?

I'd say those rounds were loaded a little hot and that the gun was probably fired quit a bit that way. S&Ws are pretty stout.

I don't. But it looks like it just cooked in the chamber. Now if it acted like a grenade, most the grip would be missing. Then I would say he definately lost some digits, and could from now on be known as lefty.

I'll hazard a guess that they were reloads. Look like Hornady hollowpoints. If the cylinder wasn't weakened from shooting a bunch of extremely hot loads quite possible the timing on the action got to be off so the bullet couldn't leave the chamber and the pressure blew apart the cylinder.
