I build change into my "routine." The way in which I change things gets altered every few weeks to few months. I usually change things because my progress has stalled, or if I happen to get bored. You have to be careful with the latter though. It's a good idea to stick with a program long enough to decide if it's actually working or not.
You hit the nail on the head for me. I usually do not stick it out long enough. I never do the same routine twice. I repeat the same exercises, but rarely in the same squence or weight. I love to superset and compound. I try to go heavy once a month on every exercise, except shoulders. I have maintained steady growth and strength, but I am trying to focus on my weaknesses. My bi's and legs are lagging at the moment.
As far as routine goes: I change very little on basic movements but I use an undulating periodization schedule which is in constant change of volume and intensity based around my 3 basic compound lifts and a variety of accessory movements with the ultimate goal of maximum strength & size.
What is your main goal with your program? How long have you been training? What did your most recent program look like, if there was much of a program?
My primary goal is to continue to add strength. I am returning from a long absence. I have back in the gym about two years.

Two days on /1 day off
Cardio 2-3 times a week.
Currently alternating Heavyand light workouts.
I am currently doing 5x5 and making some good gains. My health is the best it has been in years. I use to travel extensively. Really hard on the body.
