I think you'd be safe to tell the doctor as a civilian, but your picture shows you in the military. I'm pretty sure if you go to a military doctor then the confidentiality laws aren't the same and they can tell your commander if you're on roids.

Ok They was going to cut me open and let it drain but he suck the most of it out. Gave me pain meds and anitbloics. His said that am lucky it didn't spread out in the muscle he said its more then like the gear I told the truth. And he told me not to do AAS. How would they be able to determine it was "gear" or "steroid" leaking out causing an abscess. Just becasue you told them you use AAS doesn't mean the infection came from gear. It was most likely caused by the B-12 you have been using.
You haven't pinned any gear for 4+ weeks, this doesn't just pop up out of the blue 4 weeks later.

Yeah, you would know for sure if you had an infection the last 4-5 weeks. It would be obvious. Probably caused from a recent water based injection.
Have you pinned any water based meds recently other than the B-12?
