The telling point is that, even when there's no weight on the bar, you can't get into a safe position at the bottom of the lift. That means you badly lack flexibility.

After you fix flexibility you can worry about your setup. Proper setup is crucial to doing the lift safely and doing the lift well. But nobody can setup properly if they can't bend down to the bar without rounding their lower back.

maybe its a back injury too, I have to be careful when I bend to grab bar because if I bend wrong I get a sharp pain
 
Its a lot better in the new vids, you're not splitting the movement up anymore, but gtbmed is dead on in that its obvious this is mainly a flexibility issue.
 
I wish you would bend those knees more at the bottom. . That will get your hips back, back flat, and your chest out.

I know what you're saying, at least I think I do.
His hips are too high starting off.

Take a look at the I posted above, at 0:50 he points out that flaw.

Go to 1:36 and you'll see where your flaw is.
