Muscle size increase - when?

Quick question, when yous say ya change your program every so often, do ya change your routines and days(ie. what day a muscle is exercised), or just incorporate new exercises for each muscle

i don't train muscles seperatly. I train upper and lower. I change different variables:

loading parameter
rest interval
eccentric rep speed
static contraction at peak contraction
static contraction at stretch position
exercises
load
rep ranges
proprioceptive elements of the exercise (grip, hand placement, foot placement, angles)
etc......

so many variables that can be altered.
