Well I've been training 3 years;
I feel like the number one thing that helps me is consistency; I never stop eating, never miss meals, never miss workouts.
If i could give you what I think is the key I would say it's a shitload of quality food and continually deriving better ways make the muscle actually work. A real key in my training is actually making the muscle you are targeting work and not just swinging weight around.
the importance of this cannot be understated (much like everything else you wrote above). Sadly, its something that i just realized.. never too late though i suppose.
