166, in theory, should give a slightyly higher pulse. 100 is the typical saturation dose (depending upon the persons weight) but you can still get a higher pulse out of higher doses....its just that your returns begin diminishing. For example...the pulse difference between 50mcgs and 100mcgs is MUCH greater than 100mcgs and 150 mcgs. Once you go over the saturation dose the dose response curve becomes much less steep and you get less bang for your buck so to speak.

Sub-Q, wherever youre comfortable doing so. I use a 30 guage slin pin. Pretty painless.
Dose preferably after you havent eaten carbs or fat for a couple of hours. Protein is fine. If your goal is anabolism then you can begin eating protein right away after you dose and add carbs and fat 20 minutes or more later. If fat-loss is your goal then the longer you wait to eat the better.
