This is true. I see some of the biggest guys in the gym with small calves. And it's not because they don't work them hard enough. You can just tell that it's genetics. But keep on working them hard, they will atleast improve some.
The only reason I don't do standing calf raises is because I get those damn red marks on my shoulders. So I do seated ones and I do calves on the leg press machine as well.
I get those marks too from the standing calf raise machine. I hate it. I very seldom do those now because of that reason. Usually takes 3-4 days to go away.
When doing leg presses my feet go pretty high on the plate so they are always above my knees anyway.
I don't see any problem with doing both squats and presses but if you have a really good and heavy squat session then I wouldn't see the need to do presses in the same workout. I think about it...

I love doing kick backs. It's one of those exercises that really hits the tricep well for me. Skull crushers don't do it for me.

I agree that you aren't eating enough. I can almost guarantee you that if you start eating better and get your ass to the gym that you will see results in a short period of time. You already look pretty good so you dont have too far to go.
